依據「學校衛生法」第8條規定，學校應建立學生健康管理制度，定期辦理學生健康檢查，並作成紀錄，主要為測量學生之健康狀況、早期發現缺點及疾病，以謀早期矯治、養成個人重視身心健康之觀念、態度與行為、增進家長與教師對於學生健康之注意與關心。
According to Article 8 of the "School Health Law," schools should establish a student health management system, conduct regular health checks for students, and keep records. The main purposes are to measure students' health status, identify deficiencies and diseases early, seek early correction, cultivate personal awareness, attitudes, and behaviors that value physical and mental health, and enhance parents' and teachers' attention and concern for student health.
一、檢查方式（由衛生福利部臺中醫院到校實施）：
一、Inspection method (Implemented by the Taichung Hospital of the Ministry of Health and Welfare at the school):
       1.健檢對象：各學制新生、轉學生及復學生
       1.Health check-up targets: new students of all academic levels, transfer students, and returning students
       2.健檢費用：715元/人，參與來校健檢當日由受檢者直接繳交於衛生福利部臺中醫院收費櫃台
       2.Health check-up fee：NT$ 715 yuan per person, On the day of the health check at the school, the examinee shall directly submit the
         payment at the billing counter of Taichung Hospital, Ministry of Health and Welfare.
       3.攜帶證件/文件：

          （1）受檢者身分證/健保卡/居留證/護照（醫院櫃檯報到時須查驗）

             be presented for verification at the hospital registration counter)
          （2）新生EZ come系統列印之「健康資料自填項目」

二、檢查須知：

        1.檢查前請先到「新生EZ come系統」完成「健康資料自填項目」，完成後請列印並於檢查當天帶到惠蓀堂1樓學校報到櫃台。
        1. Before the examination, please first go to the "New Student EZ Come System" to complete the "Health Information
           Self-Filling Items". After completion, please print it out and bring it to the school registration counter
           on the 1st floor of Huisun Hall on the day of the inspection.
        2. 檢查前需禁食6-8小時（可喝少量白開水），前三日請勿劇烈運動、熬夜、抽煙、喝酒，以免影響檢查結果。
        2. Fasting for 6-8 hours before the examination (you may drink a small amount of plain water). Avoid vigorous exercise,
           staying up late, smoking, and drinking in the three days leading up to the examination.Alcohol, to avoid affecting
           the test results.
        3. 視力檢查：有配戴眼鏡者，請攜帶眼鏡以便測量矯正後視力。
        3.Vision test: For those who wear glasses, please bring your glasses for measuring corrected vision.
        4.尿液檢查：用塑膠杯截取中段尿，再倒入尿液試管。如遇生理期仍需做尿液檢查，繳交檢體時請告知工作人員另做註記。
        4. Urine test: Collect midstream urine in a plastic cup and pour it into a urine test tube. If a urine test is needed during menstruation,
            it should still be done.When submitting the sample, please inform the staff to make a separate note.
         5. X光檢查：請穿著無鈕釦、無金屬、無亮片的上衣，如有懷孕，不需要做X光檢查。
         5. X-ray examination: Please wear a top without buttons, metal, or sequins. If pregnant, X-ray examination is not necessary.
三、無法於預約時間完成健康檢查者：
三、Those who are unable to complete the health check-up at the scheduled appointment time：
       1.115學年度得標醫院（衛生福利部臺中醫院，享合約價格）：
       1. Hospitals Awarded in the 115th Academic Year
         (Taichung Hospital of the Ministry of Health and Welfare, enjoying contracted prices)：
          （1）2026/9/9~9/30在預約名冊者，不需要提早預約，直接列印「新生EZ come系統」之「健康資料自填項目」拿到醫院補檢，
                  報告統一送學校健康及諮商中心。
          （1）For those who are listed in the appointment book from September 9, 2026, to September 30, 2026., there is no need to
                   make an early appointment. You can directly print out the "Health Information Self-Fill Items" from the "New Student
                   EZ Come System"and bring it to the hospital for supplementary examination. The reports will be sent to the school's
                   health and counseling center.
       （2）自115學年度起，健康檢查報告將調整如下：於2026年9月30日前至得標醫院完成檢查者，醫院將製作紙本總報告提供校方留存；
               自2026年11月起,受檢人可使用個人帳號及密碼登入系統查詢個人健康檢查結果。若需申請衛生福利部格式之醫院健檢報告正本，
               須由受檢人本人或持醫院委託書至醫院櫃檯辦理，並酌收工本費每份新臺幣120元。
        （2）Starting from the 115th academic year, the health examination report will be adjusted as follows: For those who complete
                the examination at the contracted hospital before September 30, 2026, the hospital will produce a paper summary report
                to be provided to the school for record-keeping; from November 2026 onwards, examinees can log into the system using
                their personal account and password to check thei  individual health examination results. If an original hospital health
                examination report in the format of the Ministry of Health and Welfare is required, it must be applied for in person by
                the examinee or by presenting a hospital authorization letter at the hospital counter, and a processing fee of NT$120
                per copy will be charged.
          （3）自2026年10月1日起，請自行列印「健康資料卡」，填寫第一面個人資料後，攜帶至得標醫院進行健康檢查。檢查報告約需
                  14個工作天完成,屆時可使用個人帳號密碼至系統查詢；如需紙本報告，可另付郵資請醫院協助寄送。
          （3）Starting from October 1, 2026, please print the "Health Information Card" yourself, fill in the personal information on the
                  first page,and bring it to the awarded hospital for a health check-up. The examination report will take approximately
                 14 working days to complete. You can then use  your personal account and password to check the results in the system.
                 If you need a paper copy of the report,you can pay the postage fee separately and ask the hospital to assist with mailing it.

                   週一至周五 下午13：30–16：00
                   Monday to Friday from 1:30 PM to 4:00 PM
     2.至非簽約醫院如：大里仁愛醫院、中國醫藥大學附設醫院或其他醫院等完成健檢，費用約1,000 ~ 2,300元不等
      （醫院等級不同，收費標準不同），並於完成健檢後，將健康檢查報告及學生健康資料卡正本一份繳交至
       惠蓀堂1樓健康及諮商中心。
     2.Complete the health check at a non-contract hospital, such as Dali Renai Hospital, China Medical University Hospital, or other hospitals,
         with costs ranging from approximately 1,000 to 2,300 NTD (the fees vary depending on the hospital's level). After completing the health
         check, please submit the health check report and the original student health information card to the Health and Counseling Center on
         the 1st floor of Huisun Hall.New students who need to apply for a leave of absence on the day of registration for special reasons are
         exempt from undergoing a health check.
四、新生如因故須於註冊當日隨即辦理休學者，可免做健康檢查。

五、休學者須於復學時完成健康檢查並繳交檢查報告至本校健康及諮商中心，始完成復學程序。
