  1. 申請方式：欲申請計畫者，請至衛生福利部官方網站(https://www.mohw.gov.tw/mp-1.html）之公告訊息區下載相關申請規定，並請備妥申請計畫書一式8份(其中1份不裝訂)、電子光碟(含word及PDF檔)2份(含歸檔1份)，內含公職人員利益衝突迴避自主檢核表(請填表人簽章)，於115年11月23日(一)上午10時前送計畫業務組備函辦理，逾期恕不受理。
  1. Application Method: Those wishing to apply for the program should download the relevant application regulations from the announcement section of the Ministry of Health and Welfare's official website (https://www.mohw.gov.tw/mp-1.html). Please prepare eight copies of the application form (one of which should be unbound) and two electronic discs (containing both Word and PDF files, one of which should be archived). The discs should include a self-assessment form for public officials to avoid conflicts of interest (please have the applicant sign and stamp the form). These documents should be submitted to the Program Affairs Division for processing by 10:00 AM on Monday, November 23, 2026. Late submissions will not be accepted.
